Editor comment: I wondered at the time they launched this space crane if we would ever post an accident for it on this site. Well, this is it, it happened. Hey folks, that's our crane up there in space, built with our tax dollars.

06/06/01: Napa, California — It is reported that Foundation Pile lost a Link-Belt LS338 late last week.

Apparently they were driving pile with the crane on a railroad trestle and pulled the machine over. Reportedly, it sat in salt water for 5 days until they could re-engineer the trestle to support a 250 ton capacity truck crane to fish it out. They dug down 30' into the mud and have yet to find the counterweights! Nobody was hurt, I'm told the operator bailed before the machine went over. No firm reason as to the cause yet.

A prior report came in from a supporter who, while driving on highway 37 near Napa, California on 5/26/01 witnessed a Link-Belt HC268 owned by Bigge Crane picking up a Link-Belt crawler crane out of the water near the above mentioned railroad trestle.

06/20/01 update: Crane tipped over while attempting to set a 48,700 lb concrete wall panel. The incident happened on June 5th.

The lattice boom failed in the center and folded to the left side. It narrowly missed two men who had no way of escape, with the boom coming down on one side of them, the cables and rigging in another direction. A 28,000 lb panel which was braced and sitting on its foundation was knocked loose by the original wall panel and began to fall in on the men. As the photos show, the crane boom folded at the perfect angle to catch the falling panel (32ft tall), and break it in half, before pushing it out of the way. The cause of the accident is still unknown.

The crane is actually owned by Continental Crane of Albany, Oregon, but used to be called Coast Crane. The crane is an old model 100 ton Link-Belt.

It is reported that a 40-ton mobile crane was damaged Wednesday after it fell 40 feet to the ground.

Reportedly, the mobile crane fell when one of several slings attaching it to a larger Gantry crane was loosened.

9/4/01 update: Safety Lessons Learned

Date: July 26, 2001
Incident Description:
Failure of KEVLAR slings while lowering a small mobile crane from the Turbine Deck to the crane bay.

Nature of Injury and Part of Body:
There were no injuries in the incident, but as you can see in the photos the mobile crane was totaled.

Conditions - Time of Day, Weather, Lighting:
This incident happened inside a Turbine building. Neither the weather nor the lighting had any affect on the incident. The time of day was approximately 10:30 P.M.

Causal Factors:
The employees were using slings that were rated 4-5 times the working load and did not notice any damage to the slings. These slings are not like wire rope slings were you can visibly see the damage nor can you see the red thread that is sewn into the nylon slings.

Corrective Actions:
All KEVLAR slings need to be inspected by a person that has the rigging expertise to know when these slings should be taken out of service.

Comments:
This Lesson Learned is being distributed to heighten awareness because several contractors that are using KEVLAR slings while lifting equipment. Links to a few photos of the incident are below. There are some KEVLAR slings that do not have what is called a tattle-tail string that shows the owner when the slings have been over loaded. Please check to see if there are any Kevlar slings in use and notice there is really no way to visibly see if the integrity of the sling is still good if the cover is not damaged, or if their Kevlar slings have tattle-tail strings for overload indication.

The following photo's are from the incident that totally destroyed the mobile crane.
