ATTENTION! The numbers above indicate there is a great need for more training and safety awareness. Anyone having anything to do with cranes should take heed. The majority of the accidents were caused by human error. Translation: smart people doing dumb things.
Enhanced Reports — November 2000
11/30/00: Arizona — A construction worker was killed on Wednesday when a 10 ton crane he was using to drill septic tank holes toppled over and crushed him in the trench. (operator error)
As of late Wednesday night his body was not removed from the trench. Possible cause was improper setup of crane on soft unstable ground without sufficient mating for the outriggers. OSHA was on the site investigating.
This is not the first accident the company has had. The last one, a crane drilling attachment contacted power lines.

11/28/00: — On Friday November 3, Maxim Crane Works (Anthony Crane) lost a jib out of a model 777 Manitowoc lattice boom crane. The accident happened when the operator tried to stand a column weighing around 30,000 pounds, off of an eighty foot jib causing a structural failure of the jib as a result of lifting an overload.
The accident occurred at the North American Stainless expansion in Ghent, Kentucky and was working for a company out of Spain.
There were no injuries reported.

11/23/00: On 11/11/00
A Crane Clipped Power Lines, Stopping Traffic
No Injuries Were Reported As Repairs Were Made Near U.S. 68.
This is power line accident number 23 reported to this site this year. All were avoidable. This is the 2nd power line accident in two weeks on this project. The crane belongs to Guy M. Turner Inc. of Greensboro, NC.

11/18/00: Torrance, California
A Maxim Crane Works 888 skip basket got hung up on something and showered material through a unit as a direct result of miscommunication of radio signals.
11/18/00: The accident happened in the Exxon Mobil Torrance Refinery.
What Happened?
The accident happened when a basket of scaffold tubing that was being lifted by a crane to the top of the FCC flue gas structure became caught on a "V" cross member. As a result, the entire load of tubing spilled from the basket and injured three contract workers.
The crane operator was working in the blind, thus radio signals were the only mode of contact with the operator.

11/17/00: There was an accident on Thursday, November 16th in Austin, Texas involving a 150 ton Link-Belt conventional owned by Alamo Crane Service of San Antonio, Texas. They were setting tilt-walls when the crane turned over. The crane was in the process of walking a tilt wall on an already congested job. The operator was in the process of letting the crane down off of the outriggers when the boom apparently buckled turning the crane over. No one was hurt in the accident.

11/17/00: Houston, Texas — Power line accident #22 reported to this site this year.
A young rigger was electrocuted after being warned about the lift cable getting to close to the power lines. Witnesses said, "He grabbed the headache ball and pushed it into the power lines." |
11/15/00: Here are some photos of an accident which occurred on November 9, 2000 in New Zealand.
The 20 tonne truck mounted Grove was lifting a Phoenix palm tree when the accident happened. The pictures tell the story.
Fortunately there were no injuries, however the house whose owner had just left on holiday was in bad shape. The crane was recovered by another crane company using a 30 tonne Kato and
recovery vehicle.

11/14/00: Fort Lauderdale, Florida — A construction crane collapsed at a riverside site just before noon today, blocking traffic on both sides of East Las Olas Boulevard in the downtown.

11/14/00: Crane boom goes over backwards at Bechtel Corporation's fifth office building presently under construction in Frederick, Maryland.
It is reported that a strong gust of wind caused the long boom to go over backwards. Fortunately, no one was injured. There was an article and photo of the accident in The Frederick Post, but as of today their Web site makes no mention of the accident. The general contractor on the project is Manekin, LLC.

Crews were erecting a tower for Western Wireless when the boom of a crane collapsed. The cab where the operator was sitting twisted off and crashed and 28-year-old Bobby Warren King of Odessa, Texas was trapped inside.
